My medical records

As a patient or the legal guardian of a patient you are entitled to ask to see your medical records. At the MSG we look after your outpatient notes in our medical records department, a secure environment at Alexandra House.

Any hospital treatment and results including xrays are recorded in your inpatient notes which are held at the Princess Elizabeth Hospital. To request your inpatient notes please contact the medical records department at Health and Social Care, Princess Elizabeth Hospital, Le Vauquiedor, St Martin’s, GY4 6UU.

To request your outpatient notes please complete the Outpatients notes request form. We aim to provide these to you within 30 days.

We will provide the medical records as photocopies or by email upon request. However, if this involves a disproportionate effort on our part (because of the volume of the notes), then we will let you know and give you the following options:

  • Wait for longer than the standard 30 working days
  • Choose a data range or specific area from your notes
  • Visit Alexandra House and view your notes personally under the supervision of a member of staff.

Please note that we will require a photocopy or a scanned image of either your driver's licence or your passport as proof of identity to release your notes.

While we are happy to provide you with copies of the information you request, we reserve the right to charge a flat fee of £25 if the request is considered to be ‘manifestly unfounded or excessive’. We will make every effort to provide you with a satisfactory form of access or summary of information if suitable.


Data subject access request

A data subject access request relates to all information held about you by the MSG. This includes emails, finance department records, and a copy of your patient registration form if we have one on file.

To make a request please complete the Data subject access request form. 

Please note that we will require a photocopy or a scanned image of either your driver's licence or your passport as proof of identity to release your notes.

The details you provide on the form may be used to update the records we hold, if appropriate.
